Format

The first row must be a header. Required columns: First Name, Last Name, plus Email or Phone. Optional: Role.

Valid Roles
Direct Care Worker (CNA, HHA)
Nursing Staff (LPN, RN)
Support (Dietary, Laundry, etc.)
Admin / Site Champion
Social Work
Other
Notes
  • Duplicates (same email or phone) are skipped.
  • Invalid roles default to "Other".
  • Programs are auto-assigned by role.
